I authorize easyGENETICS (EG) to collect a biological specimen from me or from an individual for whom I have the legal right to authorise a collection. I further authorise EG to perform DNA testing on that specimen for the purpose of genetic predisposition testing. If this test is for a minor or person who is incapable of giving consent, I attest that I have the legal authority to consent for the collection and DNA testing of the biological specimen of that person.

All materials and products provided by EG are provided for informational and educational purposes only.  It is advised to consult with a physician, or health care provider regarding any questions pertaining to the results of this test. This service does not replace a visit to a physician, nor does it replace the advice or services of a physician. 

I agree not to hold EG liable for any damages caused as a result of the use of the information or products contained herein. I further agree to release, indemnify, and hold harmless EG and its officers and employees from all liability for my use of this product(s), and I agree not to hold EG liable for any action that might be taken by users of this service.

I confirm have read all the information provided by EG regarding this test and have had the opportunity to ask questions I might have about the testing, the procedure, the risks, and the alternatives prior to my informed consent.

PURPOSE OF TEST: The purpose of the molecular genetic test is to ascertain if I, my child or an individual for whom I have legal rights to represent, are carrying mutation(s) predisposing to or causing the specific diseases or conditions covered in the ordered test.

TEST RESULTS: I understand that due to the complexity of DNA based testing and the important implications of the test results, I am responsible to obtain the necessary professional pre and post-test counseling.

I understand that EG recommend that results should be reported through a suitably qualified health care provider. I therefore consent to EG to send my results to the designated health care provider and I must contact my provider to obtain the results of the test.

However, if I elect to receive the results to my personal attention then I assume full and final responsibility to discuss the result with a suitably qualified professional and obtain the necessary guidance.

The test results, in addition, may be released to any person or entity that may be legally entitled to such data.

I understand the limitations of these results: the test results are based upon probabilities, and may not provide a 100% definitive conclusion to either genetic disease predisposition or manifestations.

EG RIGHTS: Refuse testing if this Consent form is not completed correctly.

USE OF SPECIMENS: After testing is completed, I understand that my blood, body fluid or tissue specimens may be disposed of or retained indefinitely for research (by your choice). I understand that no compensation will be given nor will funds be forthcoming due to any invention(s) resulting from research and development using the specimens submitted. I understand that I may refuse to submit my specimen for use in this way and may withdraw my consent at anytime by contacting EG. I understand that my refusal to consent to medical research will not affect my results. I have indicated my consent or denial on the submission and consent form received with the kit. Failure to indicate my preference will be taken as an acceptance to use my samples.

REFUND POLICY: easyGENETICS provides the following refund policy and once you have ordered your purchase is subject to the following conditions:

Refund of full price of purchase can only be made before your kit has been dispatched. You must contact us by email and request cancellation of your order before the kit has been sent out to receive a full refund.

Refund of purchase price minus administration kit fee after we have dispatched your kit and within 7 days of your order being placed, you may contact us by email and request cancellation of your order. A refund of the purchase price will be given to you less a €65/£60/A$100/C$100 administration fee if we are contacted within 7 days from when your order was placed.

No refund can be given if it is over 7 days from the date of your purchase or if your samples have been returned to us and have been received at the laboratory.
